The post holder will work with the Head of Continuing Care to define and agree local continuing care policy and process in collaboration with key stakeholders, facilitating discussions and advising how best practice will be delivered operational and working collaboratively with key stakeholders. Deputising for the Head of Continuing Care as and when required.
